How to Choose your QExA Health Plan

QUEST Expanded Access (QExA) is a new Medicaid program for seniors, 65 and older, and people of all ages with disabilities. Starting on February 1, 2009, these clients will receive their healthcare services through a QExA health plan.

Your health is important. Choose a health plan that meets your needs.

Near the end of September 2008, we will send QExA clients a New Enrollment Packet. Look for a bright yellow envelope. It has information on QExA, the health plans, and how to choose your health plan.

Take an important step toward healthy long-term living by choosing your health plan.

Step 1: Learn About Your Choices

It’s easy to learn about QExA and your health plan choices. To find out more you can:

Call us toll-free at 1-866-928-1959 and speak with an Enrollment Counselor. We are here to answer your questions and help you choose your health plan.

Attend a QExA meeting in your area. Enrollment Counselors will provide information on QExA and your choices. You can get all your questions answered and choose your health plan, too. Call us toll-free at 1-866-928-1959 to find a meeting near you.

Meet face-to-face with a QExA Enrollment Counselor. To make an appointment call us toll-free at 1-866-928-1959.

Step 2: Choose Your Health Plan

With QExA, you will receive medical treatment through your health plan. This means you visit the doctors, hospitals, and care facilities that work with your health plan. You will choose a doctor, called a Primary Care Provider (PCP), who will be responsible for your medical care. If you don’t have a doctor, your health plan will help you find one.

Here are some questions to ask before you choose a health plan:

Do you have doctors or providers you want to continue to see?

Do you have a hospital, care facility or pharmacy you want to use?

Call us toll-free at 1-866-928-1959 to find out which health plans they work with.

Look at your health plan choices and find one that:

Lets you stay with the doctors you use now.

Provides the services you need.

There are two QExA health plans: Evercare and ‘Ohana Health Plan. If you don’t choose a health plan by December 1, 2008, one will be chosen for you.

Step 3: Sign Up with Your Health Plan Choice

There are three ways to sign up with the health plan you choose.

Send us the QExA CHOICE Form from your New Enrollment Packet. Fill it out and mail it to us at:
